What are your office hours?
Patients may schedule an appointment by visiting the between 8:00 AM and 5:00 PM Monday – Friday or by calling 239.201.2264. For medical emergencies, dial 911.
What happens if I need to cancel my appointment?
Sometimes the unexpected happens and you need to cancel your appointment make sure you call us or email us at least 24 hours prior to your appointment date.
What should I bring to my appointment?
First appointment, please bring the following:
Insurance card
A picture Photo
Any previous medical records (including test results from other clinics)
All current medications (with prescription )
Repeat Patient Appointments

Do you provide transportation?
Yes. However, transportation is provided on a case by case basis. If you need transportation to your scheduled appointment, please call our office.
